Outfits & manual garments

Outfits automatically contribute a deduplicated garment list, with one unit per garment by default. Removing an outfit removes only garments no longer used elsewhere. Manual selections, extras, quantities and packed settings are kept; settings return if a garment is re-added. Later outfit edits update the list automatically.
